2010-2011 Goals & Indicators
for Participant Outcomes
1. GOAL: Obtain and Remain in Stable Housing
INDICATORS OF OUTCOME ACHIEVEMENT:
65% of participants will obtain stable housing at CLP program exit; with 100% of those completing the program being stably housed.
- a. Upon exiting the CLP program, 65% of participants will obtain stable housing; with 100% of those completing the program being stably housed.
- b. 75% of CLP graduates remain in stable housing for at least one year.
- c. 80% of non-residential participants will obtain and/or remain in more stable housing.
2. GOAL: Achieve Greater Self-Determination
INDICATORS OF OUTCOME ACHIEVEMENT:
- Before program exit, 85% of participants will meet at least one goal on their individual service plan.
- After program involvement, 85% of participants will be less isolated, as measured by the number of community resources and services they are accessing.
3. GOAL: Improve Teen Parent Health, Development & Well-Being
INDICATORS OF OUTCOME ACHIEVEMENT:
- During program involvement, subsequent pregnancy rates for participants will be lower than national rates for teens in comparable groups.
- During program involvement, rates of abstinence and contraceptive use for our participants will be higher than national rates for teens in comparable groups.
- Before program exit, 90% of participants will be enrolled and seeing a medical provider for preventative and tertiary health care.
- 100% of participants who are assessed as needing mental health services will be referred and encouraged to participate in treatment before program exit.
4. GOAL: Improve Child Health, Development & Well-Being
INDICATORS OF OUTCOME ACHIEVEMENT:

By age two, participants’ children will be up-to-date with immunizations at higher than the national rate for children from comparable groups.
- Pregnant program participants will deliver fewer low birth weight babies than national rates for comparable groups.
- Before program exit, 90% of participants’ children will be enrolled and seeing a medical provider for well-child preventative and tertiary health care.
- 100% of participants’ children who are suspected of having developmental delays will be referred for diagnostic assessment and treatment.
5. GOAL: Build Healthy Parent-Child Relationships
INDICATORS OF OUTCOME ACHIEVEMENT:
- In follow-up parent efficacy screenings, 80% of participants will show improved parent-child relationships in follow-up parent efficacy screenings.
- During program involvement, rates of indicated child abuse/neglect will be lower for our participants, after program involvement, than national rates of teens in comparable groups.
6. GOAL: Build Basic Life Skills through Education & Training
INDICATORS OF OUTCOME ACHIEVEMENT:
- By program exit, 50% of participants will have increased their financial literacy.
- By program exit, 75% of participants assessed for computer skills and proficiency will possess 8 out of 10 basic skills before program departure.
- Before program exit, 90% of participants will receive instruction to develop public speaking and presentation skills.
- 90% of Finishing Academy graduates will be placed in employment internships within one week of Finishing Academy completion.
7. GOAL: Achieve Enhanced Self-Sufficiency
INDICATORS OF OUTCOME ACHIEVEMENT:
- During program involvement, 50% of participants will be enrolled in programs or schools to obtain high school diplomas, GEDs, vocational certificates, or post-secondary education degrees.
- During program involvement, 0% of participants will obtain/retain employment.
- By program exit, 20% of participants will use less public assistance after program involvement.
8. GOAL: Instill in Teen Parents an Attitude of Civic Mindedness
INDICATORS OF OUTCOME ACHIEVEMENT:
- Each year, 12-20 current participants will serve a 3-month term on the participant Leadership Council each year.
- Each 3-month term, 75% of Leadership Council members will volunteer as mentors for current participants and/or participate in other community service activities.
